Fill out our online request form and we can call you direct or email you as you prefer.



Payment & Fees

Crossroads offers a sliding fee scale based on income. Please let us know when you contact us by phone or when you submit your online request whether you would like to take advantage of the sliding scale. Payment is expected at the time of service, and there is a 24-hour cancellation policy.



Cancellation Policy

Please let us know if you can't make your appointment. A 24-hour advance notice is required for the cancellation or rescheduling of all appointments.
